Job Description:
Come join our awesome team as a Physical Therapist with Home
Health of Western Colorado. This is a PRN role! Must have Pediatric
experience. Home Health of Western Colorado is part of Volunteers
of America National Services which serves as the Housing and
Healthcare subsidiary of the Volunteers of America parent
organization. Pay: $54.52 - $58.42 per hour based on experience
Schedule: This is a PRN role and requires some travel to client's
homes The Physical Therapist will provide ongoing therapy
evaluations and treatments and education to clients in a timely
manner per physician’s orders. To provide leadership and
supervision of physical therapy services to other physical
therapists, assistants and aides. The Physical Therapist evaluates
and treats clients, communicates with families, physicians and
other health team members and maintains appropriate documentation
of services and complying with State and Federal regulations.
Participates in on-going agency meetings and conducts in-service
and training for agency staff on ongoing basis. QUALIFICATIONS:
Graduate from a physical therapy curriculum approved by the
American Physical Therapy Association and is currently licensed as
a Physical Therapist in state where practicing. Able to understand
medical terminology and how it applies to physical therapy, to
communicate with residents, staff, physicians, and to prepare
meaningful reports. Spatial and form perception are necessary to
visualize anatomical structure of the disabled part of the body and
the relation to therapeutic devices in remedial actions.
Coordination and dexterity are necessary when treating clients and
handling therapeutic equipment. Ability to communicate with clients
to establish favorable attitude and to motivate the clients to
desired cooperation and effort in his/her rehabilitation
development. Possesses adequate strength and endurance to meet the
physical demands involved in moving clients and equipment into
position for treatment and in manipulating equipment and clients.
Be willing to work weekends and holidays, as needed, on a
rotational schedule. Be willing to travel and work at other VOANS
facilities, as needed. If operating company vehicle you must be
twenty-one (21) years of age and meet the MVR organization
standards. Maintain productivity standards as directed by
supervisor. Ability to utilize computers and other electronic
devices for tasks such as timekeeping, in-servicing and
documentation. ESSENTIAL FUNCTIONS: Accurately screens and
evaluates clients and provides PT upon written order of physician
within twenty-four (24) hours of referral. Evaluates each client
and initiates any ordered treatment(s). Maintains a line of
communication with appropriate agencies for prior and continuing
authorization of therapy treatment. Communicates a report regarding
the client’s progress to attending physician. Consults with the
physician regarding clients’ change of condition as indicated
during the course of treatment. Re-evaluates the client in
conjunction with the attending physician. Supervises the operation
of physical therapy services to ensure an acceptable level of
performance from assistants and support staff. Prepares and
maintains treatment records with evaluation and progress notes,
including: Physician’s orders. Objectives of treatments. Plan of
care. Treatment procedure(s) and baseline clinical data. Evaluation
of progress and client's response to treatment. Other pertinent
information as required by agency's policies. Supervise physical
therapy assistants and/or physical therapy aides in direct client
care and client-related activities according to State and Federal
requirements. Counter-signs documentations per regulations.
Participates in client care planning conferences, initial
conferences, for residents on physical therapy programs, and when
requested by nursing and/or therapy department. Participates in
other agency meetings as directed by Director of Clinical Services.
Communicate with Director of Clinical Services and other health
team members regarding client progress, problems and plans for
specific clients. Confers with Director of Clinical Services on an
ongoing basis relevant to the development and implementation of
department objectives with respect to overall agency and
organizational goals. Maintain productivity standards as directed
by supervisor. Assists therapy assistants in maintaining
productivity standards. Maintains proper documentation and billing
functions using computer. Assists with personnel functions of the
Physical Therapy Department along with Director of Clinical
Services: Develops, implements, and maintains work routines and
staffing schedules. Initiates and directs in-service training
programs for the department. Teaches appropriate physical therapy
procedures and supportive activities to clients, family members (as
needed), and other health care personnel who share responsibility
for client's care in a variety of settings. Teaches principles of
correct body mechanics to all employees as scheduled by the Staff
Development Coordinator. Assists in ensuring that physical therapy
services are provided on a continuous basis without interruption.
This includes finding supportive personnel if regular employee is
absent due to vacation, sickness, leave of absence, or other
reason. Assists with finding replacement for therapist/assistant
when needed. Attends required in-services & completes assign
on-line modules Comply with the Physical Therapy Practice Act in
the state. Performs other duties or special assignments as directed
by the Director of Clinical Services. At Home Health of Western
