This position is part of a rapidly growing program and will assist with care coordination, research studies, provide patient and family education, scheduling, triage, assist with procedures, and collaborate with a multi-disciplinary innovative care team committed to innovation and improving patient outcomes. This is a high volume fast-paced Magnet designated clinic. This clinic is INCP and QOPI Certified. HOURS: Monday - Friday 8 AM to 4:30 PM. Responsibilities: Phone Triage Schedules emergent-need appointments. Patient follow-up calls. Patient education on particular procedures and processes. Subject matter expert in respective specialty for hospital RNs in the units. Regularly meets with other disciplines to share best practices on care treatments. Care for patients with a long-term treatment process. Job Requirements Applicable Experience: 2 years Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various Associate Degree Job Details Full Time Day (United States of America) The best place to get care.
